By confirming your appointment with us, you are agreeing to the following statements:
I have not returned from overseas travel (or been on a cruise) in the last 14 days.
I do not have any flu-like symptoms such as fever, cough, sore throat, nasal congestion, runny nose or shortness of breath
I have not had close contact with a COVID-19 positive case in the last 3 weeks
If I have answered no to any of the above statements, I understand that clearance by a medical practitioner is required and proof of this may be requested
If any circumstances change prior to your appointment, please notify our team immediately.
As the Australian government is regularly reviewing guidelines for how small businesses operate, we ask for your understanding. This means there is a possibility we may need to contact you to reschedule your appointment.
